ALL EMPLOYEES
All Employees of the Diocese of San Bernardino, whether full-time, part-time,
contract, or religious, must complete a Live Scan background check.
If you receive a salary or stipend for your ministries, you are considered a employee.
You will only need to complete your personal information, along with an identifying number (YOUR NUMBER) linked to the location you work.
Each school or parish has an unique code which you must put in the "YOUR NUMBER" line. If you serve both a school and a parish, or multiple parishes, for instance, you can include the code for all these locations in the "YOUR NUMBER" line.
To Download: Select the appropriate link, below, to open the Live Scan request form with the required Department Codes already filled in for you. Alternatively, you can obtain your livescan request from the school or parish office where you work.
