The request forms for the Live Scans for priests and deacons are not available online.
Moreover, priests and deacons MUST NEVER USE the Live Scan request forms from their parishes,
nor the parish or school entity numbers.
They will risk having to repeat their Live Scan. Instead, the proper Diocesan Office,
below, will provide the Live Scan Request Forms.
NOTE: As religious brothers and sisters are not clergy, these instructions and entity number DO NOT APPLY to them, whether they are living within a religious congregation, or they have assignments in the Diocese as Pastoral Coordinators, for example.

DIOCESAN PRIESTS:
- The request forms for priests' livescan are not available online.
- All livescan (digital fingerprinting) form requests will be provided by the Office Administrator of the Office of Priest Personnel. 909-475-5123
- Properly-filled forms will have 1609P in the YOUR NUMBER line.
- NEVER use the entity code of the parish to which you are assigned, or you may have to redo your Livescan.
RELIGIOUS PRIESTS:
- The request forms for religious priests' livescan are not available online.
- All livescan (digital fingerprinting) form requests will be provided by the Office Administrator of the Office of Priest Personnel. 909-475-5123
- Properly-filled forms will have 1609P in the YOUR NUMBER line.
- NEVER use the entity code of the parish to which you are assigned, or you may have to redo your Livescan.
ALL PERMANENT DEACONS:
- Since all Diocesan Deacons are already Live Scanned, or are Live Scanned during Deacon preparation, the following would generally apply to Deacons transferring from outside the Diocese.
- The request forms for Deacons' Live Scan are not available online.
- All Live Scan (digital fingerprinting) form requests will be provided by the Office of Ecclesial Services. Properly-filled forms will have 1609D in the YOUR NUMBER line.
- NEVER use the entity code of the parish to which you are assigned, or you may have to redo your Livescan.
VISITING CLERGY:
- All Visiting Clergy who plan to officiate in liturgical celebrations, sacraments, or other events serving as a member of the clergy must contact the Office Administrator in the Office of Priest Personnel in advance regarding background check arrangements that would pertain to them. 909-475-5123.
