All Pastoral Coordinators of the Diocese of San Bernardino must complete a Live Scan background check.
Pastoral Coordinators may acquire their Live Scan request by downloading it, or requesting it from their parish office or
the Diocese Human Resources office.
ATTENTION:
Pastoral Coordinators who attain their Live Scan request from their parish should pay special attention to ORI (A3004) and mail
code (01173) numbers, because they are different from the ones typically associated with a parish.
Pastoral Coordinators are not to be confused with religious education coordinators. In accord with Canon 517 #2, and according
to the USCCB document "Co-Workers in the Vineyard of the Lord", Pastoral Coordinators are refered to as "lay ecclesial ministers."
Pastoral Coordinators will need to complete the personal information, along with
an identifying number (YOUR NUMBER) linked to the parish at which you are assigned.
Each parish has an unique code which
you must put in the "YOUR NUMBER" line (same as the entity number). If you
serve both a school and a parish, or multiple
parishes, for instance, be sure to include the code for all these
locations in the "YOUR NUMBER" line.

Make sure all the fields are filled
in correctly, or you risk having to redo the Livescan.
