The request forms for the Live Scans for diocesan seminarians and deacons-in-training are not available online.
Moreover, seminarians and deacons-in-training MUST NEVER USE the Live Scan request forms or entity numbers from their parish, school or ministry.
They will risk having to repeat their Live Scan. Instead, the proper Diocesan Office,
below, will provide the Live Scan Request Forms.

DIOCESAN SEMINARIANS (1657):
The following applies ONLY for seminarians who are preparing for the priesthood through the Office of Vocations at the Diocese of San Bernardino.

- The request forms for Diocesan seminarians' Live Scan ARE NOT available online.
- All Live Scan form requests will be provided by the Director of the Office of Vocations. (909) 783-1305
- YOUR NUMBER: 1657 - "YOUR NUMBER" line on the request, all Diocesan seminarians must use a code of 1657, which is the Office of Vocations.
- ABSOLUTELY DO NOT use the Live Scan request forms or the code number (entity) from your parish. You risk having to repeat your Live Scan.
NON-DIOCESAN SEMINARIANS (RELIGIOUS ORDER or SEMINARIANS FROM OTHER DIOCESES):
The following applies to any Seminarians who are not Seminarians of the Diocese of San Bernardino, including those preparing for the Priesthood through a Religious Order (example: Holy Spirit Missionary Cenacle, etc.)

- Live Scan (digital fingerprinting) form requests may be downloaded or provided by your director.
- YOUR NUMBER: Unlike Diocesan Seminarians, Non-Diocesan Seminarians in religious orders will use the number code of the parish, school or ministry your serve. If you receive a stipend, you are an employee. Otherwise, you are a volunteer.
